Your First Visit Guide
Here is what to expect when you visit Arthritis Care of Los Angeles for the first time.
Schedule Your Appointment
Call us or book online. Our team will verify your insurance and send you new patient forms to complete before your visit.
Complete Patient Forms
Fill out the new patient registration, medical history questionnaire, and HIPAA privacy notice. Bring them to your appointment or submit online.
Gather Your Records
Bring your photo ID, insurance card, referral letter (if required), current medication list, and any previous lab results or imaging studies.
Check-In & Registration
Arrive 15 minutes early. Our front desk team will verify your information, confirm insurance, and answer any billing questions.
Meet with the Doctor
Dr. Forouzesh will review your complete medical history, perform a thorough physical examination, and discuss your symptoms in detail.
Your Personalized Treatment Plan
Receive a customized plan tailored to your condition, lifestyle, and goals. Lab work or imaging may be ordered. All options are explained clearly.
Insurance Information
We work with most major insurance providers. Contact us to verify your specific plan.
PPO Plans
Most major PPO plans are accepted. PPO plans generally allow you to see specialists without a referral. We will verify your benefits before your visit.
HMO Plans
Select HMO plans accepted. A referral from your primary care physician is typically required. Contact us to confirm your specific plan.
Medicare / Medi-Cal
We accept original Medicare and most Medicare Advantage plans. Medi-Cal patients are also welcome. Call to confirm eligibility.
Workers' Compensation
We accept workers' compensation cases for work-related injuries and conditions. Authorization from your employer or insurance carrier may be required.
